There's no difference in how you pay when you're at the doctor's office (or vet!) or online ordering contacts, etc. The Paytient app is incredibly simple and quick to set up your payment plans, including paying it off immediately, and breaks down your plans by the amount each payment would be. As someone who has worried about paying for expensive prescriptions and medical equipment, Paytient offers me peace of mind that my wallet isn't going to take a huge hit at once, and I won't be left paying interest for having to use a credit card to cover my costs. There was virtually no set up time - my card came in the mail and I was able to use it at my next visit seemlessly.
