Jotform gives you the ability to create forms directly from a PDF as well as creating web based forms from scratch. Our team required the outcome form to be a specific format and Jotform allows you to do just that! We were also using a custom built form for our mission trips (we had to have docusign and medical form attachment). There weren't any off the shelf form builders that had this when we created this, but we have since moved all of our forms over to Jotform now that they have all of those features. Jotform is an incredible tool for allowing my clients to fill out required forms in a way that is simple, HIPAA compliant, and requires no printing/mailing/faxing. Forms are customizable and you can make an unlimited number to meet your needs. There are definitely a few things that would make it more user friendly and convenient. Sometimes the PDF output has the answers to questions seemingly out of order or formatted strangely, even though it looks correct on the form itself. Things like conditional formatting seem trickier than they should be. It would also be great to have more options for the file naming convention for the Google Drive integration, like having it appear with the name of the form and the client last name, versus a date/time stamp. I really like the versatility of JotForm. Being able to customise terms and develop templates for my projects is incredibly helpful in making the workflow efficient and smooth. The variety of widgets, form fields, notifications and integrations make it easy to tailor forms to a specific process or task. This makes life much easier when dealing with non-linear workflows that require extra attention, while still providing users with an intuitive interface they're familiar with, regardless of whether optional third-party programs have been integrated for additional support. I strongly dislike the organisation of Jotform's dashboard. Its arrangement based on recently used forms makes it difficult to find what I need quickly and easily; as a result, I have to spend extra time searching for items. Also, each time I create a new form, there is a chance that my carefully crafted formatting will be lost in translation due to the different design tools available.
