Revolutionize and Automate Mundane Tasks Using FormAssembly!
FormAssembly makes it easy to accomplish these goals. From a stellar interface to amazing customer support and everything in between. It was incredibly easy to implement into our workflow and brought us tremendous value in how we use it.
FormAssembly is used daily and many times per day utilizing patient facing questionnaires, evaluation of services, etc. In the future we plan to use it for patient scheduling. It was incredibly ease to integrate with our system. Review collected by and hosted on G2.com.

FA just makes work so much easier to complete. I love:

OMG why the hell are there no folder options? All the forms are just dumped into one big section. I have almost 100 forms and tags just don't do it for me the way organizing in folders would. Please for the love of God add folder options Review collected by and hosted on G2.com.

Form Assembly for the price is a really robust solution. Their data security is top notch, surveys and workflows are extremely powerful, and end user configuration is really quite easy to get the hang of to build and maintain with ease. With an implementation manager we were able to build out a fairly complex and robust workflow solution within Form Assembly successfully with limited support and empowering us, the customer, to own that product build out.
The tool continues to be enhanced with additional features. Requests have been submitted fro additional enahncements and their DEV team seems open to ideas, but slow to actual produce and develop. Their customer service team definitely provides exceptional support- most tickets are responded to same day within hours of their submission.
We do not have an integration with Form Assembly (were attempting to connect via an API into the workflow solution- but Form Assembly only has an API stood up for surveys, not workflow) so I cannot provide feedback from here.

We have asked for several development enhancements that we feel could be beneficial beyond just our use cases. Unfortunately none of our ehnahcement requests have been taken to the point of DEV enhancement or release.
We leverage workflows heavily within Form Assembly and although very dynamic and robust- periodically one crashes (less than 1% of the time) but when a workflow crashes for no apparent reason- there is no reason/trigger or notification... so it either has to be processed manually outside the system workflow or deleted out and restarted. Wish the glitch rate was 0% instead 0.5%.
Beyond these couple deficiencies Form Assembly is really quite good. Bang for your buck they do provide a very solid software solution. Review collected by and hosted on G2.com.

I love that FormAssembly lets us collect data from people external to our Salesforce organization without creating duplicates or exposing vulnerable information. We also make great use of their SSO security and form prefill to make employee-centric forms both secure and accessible. Review collected by and hosted on G2.com.
FormAssembly's formula writing tool can be comparatively difficult to work with, and the Response queries/reports are sometimes a little janky. Many of FormAssembly's best features are locked behind higher-cost tiers, which can create barriers for nonprofits. Still, it's more than worth it. Review collected by and hosted on G2.com.

How easy it is to create a form and map it to a salesforce data set. The Salesforce integration is my favorite part, it was very easy to set up. (I'm biased, I'm a Salesforce pro) But the customizations you can make on the form, and then how you can have those form entries trigger other salesforce automation was a game changer for our customer intake process. We also have web forms and a payment form where you can receive online payments with a web form! The customer support was always responsive and super helpful when exploring potential use cases. I can't think of any. I'm not sure if there would be any limitations on the UI or if you wanted to customize anything, but I don't have complaints. Review collected by and hosted on G2.com.
I like that we can make each form unique and customizable to our events. We can change background images, colors, just about anything we want with a few simple clicks. Review collected by and hosted on G2.com.
I wish some of the connection pieces between the form and our CRM would save across forms. Something along the lines of a basic connection setup and then have the option to tweek it as needed would be wonderful! Review collected by and hosted on G2.com.

I have always appreciated the support team at FormAssembly. Beth, our account rep, is great to work with. The Dev team is always improving their product with new and improved features.
