Looking for a Survey tool that can be integrated with a new Web application (java, jquery, javascript, apache, tomcat, etc)

We are building a new application and need to embed surveys into the tools. We are looking for a survey software that we might be able to use to build and deliver the surveys from within our application. Our use case is in medical management, we would have nurses on the phone asking members questions from the survey, not emailing the survey to members. Does anyone have any suggestions of a survey tool they've done something like that with? Thanks
