That is cloud, opensource if you wanted. Formio has libraries for all major front-end frameworks. WE are currently in the process of using the docker solution it provided for better security.
The editor is awful! We have some forms with lots of questions, and we can't insert new questions in the middle of the form. We have to move all the rows below the new question one by one. Not every time, but sometimes when we save a form, it doesn't save....

This is only a tool for simple forms and surveys, which is great in a pinch but you can't do advanced survey logic within it a la Qualtrics.
