**What do you like best about Spruce Health?**

I love that I can connect an automatic function to it. I used to spend about four hours a week sending out cancellation emails. I'm a mental health therapist that runs my own private practice, but with Spruce Health's Communicator plan and API integration, I was able to create an auto text feature using the Keragon AI workflow program. It has saved me a ton of time and mental energy. Very grateful to have found Spruce Health!

**What do you dislike about Spruce Health?**

There's a little bit of a learning curve with all the different options, but because the support has been great, it hasn't been too much of a hurdle.

**What problems is Spruce Health solving and how is that benefiting you?**

I love the API integration. It's allowed me to set up an auto-texting feature to notify people on my wait list when I have openings. It's saving me at least four hours a month.

**What do you like best about Spruce Health?**

Ability to both have messaging and telehealth visits.  Ability to collect payments using linked Stripe account.  Generally high quality communications as long as both parties have adequate bandwidth.
Easy to set up.
Easy to have away messages. 
I essentially use this app daily.

**What do you dislike about Spruce Health?**

Reliability and difficulty some patients have receiving calls (patients have told me, for example that they hear no ring and saw no pop-up, despite obviously being signed in.)
  Customer service is not always helpful. 
 Lack of ability to have three way calls (making it hard, for example, to involve language interpretation and to have a patient's family member connect if they are not in the same location.) 
Some features which would be helpful, such as screening non-urgent messages to not alert me in the middle of the night (such as payments and requests for refills and appointment confirmations) require an expensive upgrade, that all participants must accept, making cost go from affordable to not affordable.
  Some patients have older phones with little storage space making it hard for them to download the application and therefor would be easier for them to use a service such as Doxy.

**What problems is Spruce Health solving and how is that benefiting you?**

Spruce hasn't really solved any business problems.  I originally started using Doxy, with free service, but found the quality lacking.  Also, the ability to collect funds is very useful, but Doxy, even the paid version, doesn't allow for messaging which Spruce does.

**What do you like best about Spruce Health?**

It's been really simple to implement and use.  I love that I can have a separate phone line on my current phone that is easily distinguishable from my personal line.  Downloading the app and using it on my personal cell was a breeze.  I had some issues with confirming my business EIN and customer service was helpful in navigating that and communicating with me along the way.  I love that clients can easily call me any time and the voicemail transcription is amazing.  I use this daily with my work and am so glad to have found an easy to use platform that fits my needs!

**What do you dislike about Spruce Health?**

My biggest gripe is that some of the features I'd really like require the Communicator Plan which is twice the price.  For a new person in solo private practice it doesn't make sense to pay that much at the moment, but setting office hours etc. would be nice.

Having a separate, HIPAA compliant voicemail/texting option for clients without having a separate physical phone.

Starting a private practice is overwhelming, but Spruce Health has been the easiest part of my setup. The interface is incredibly intuitive—I was able to get my business line and secure messaging running in minutes. It feels like it was designed by people who actually understand a clinician’s workflow.

**What do you dislike about Spruce Health?**

When someone calls my Spruce number, my phone often displays my own Spruce number rather than the patient’s name or number. This makes it hard to know who is calling before I pick up.

I was nervous about porting my business number over, fearing I’d lose connectivity or hit a technical wall. Spruce made the entire process seamless. Their tech support is fantastic—they didn't just point me to a FAQ page; they actually guided me through and ensured my number transfer went off without a hitch.

I like that Spruce Health gives so much clarity on how to set up operations. It provides a lot of different options on how I can make it automated when someone contacts the company. The phone tree is really good, as well as the set time schedule on who would be contacted, based on the time of day, vacations, etc.

I think Spruce Health could provide an included phone tree option under the basic plan instead of having to upgrade to the Communicator plan.

I use Spruce Health for a business phone within my mental health practice. It enables patients to contact the main line with a phone tree for mental health or psychiatry. It ensures HIPAA compliant communication within the company.

**What do you like best about Spruce Health?**

I use Spruce Health for my private practice, and it is a great platform that helps me connect with my clients via phone and text message. It's wonderful for secure faxing, which I utilize a lot, and it makes me feel comfortable and confident that my clients' information is secure. I really love how helpful the people are that work there, with a really quick response time, which is very helpful. If I have an issue, I can reach out to them, and they are very quick to respond. I also appreciate that when Verizon Wireless was down, Spruce was not, so I was able to communicate with my clients via the computer through text, which was incredibly helpful. The initial setup was incredibly easy, and the support was seamless.

Spruce is an asset to our practice. It allows us to communicate effectively and efficiently with our patients in a HIPPA compliant manner with the ability for our support staff to view the conversation and assist accordingly. Patients created in our EHR automatically have a profile created in spruce. Lastly, the ability to create a note in the patient’s chart from the messages assists with continuity of care and helps keep our records up to date!

Learning curve with the technology and utilizing the yealink phones.

I LOVE that I can set up auto replies to missed calls.  I own and operate my own business and when I'm with clients I can't answer the phone.  The auto reply feature has been incredibly helpful in retaining new clients for me! This app has basically paid for itself at this point.

**What do you dislike about Spruce Health?**

My only issue was the learning curve with the online portal and getting everything set up.  I needed a lot of help from support (who was very helpful) and I asked Gemini AI a ton of questions to understand call flows.

**What problems is Spruce Health solving and how is that benefiting you?**

Auto replies, staying HIPAA-Compliant, feeling more secure texting clients, away messages or out-of-office responses to texts/calls.  All of it has been amazing really.

Large-group organization, internal messaging, scheduling message feature, team chat, use without a portal, SMS/MMS sharing, after-hours responders, API sharing. Great ease of implementation for new users, the customer support is superb, and I use it every day all day long.

**What do you dislike about Spruce Health?**

I cannot think of anything that I dislike; everything is perfect!

**What problems is Spruce Health solving and how is that benefiting you?**

I run a multistate telemedicine clinic with multiple providers and support staff. I use the scheduling feature to send appointment reminders, set staff reminders, create dose-increase reminders for patients on medication titrations at home, and follow up with people who presented in crisis a couple of days later. I also prioritize patients in crisis using the star feature. Overall, it solves the challenge of coordinating a dozen staff members and thousands of patients across the US by giving us a central place to connect and stay organized.

**What do you like best about Spruce Health?**

We have been using Spruce in our private practice for 2 years now. I cannot emphasize with words alone how great Spruce has been for us. First and foremost, it’s reliable. This is so important & unfortunately that’s not always the case with other phone systems out there. It’s convenient. Texting is the new standard outreach practice. Patients appreciate the ability to text with the office on their own schedules. We appreciate the ability to set auto replies for after hours or schedule follow up touch points for a later date so it doesn’t fall through the cracks. The recent updates to self-serve phone trees has been great too. Finally, the value you get for the price is great!

**What do you dislike about Spruce Health?**

I don’t care for the lack of a phone number to call for assistance, but that seems to be the standard for a lot of companies anymore. I will say that they are pretty quick in answering any questions through the chat & we haven’t needed to reach out very often.

Spruce helps me keep my faxing, messaging, phone calls and marketing all-in-one. The staff are fantastic and helped me get my fax number from another platform. I love that my clients get to choose what works best for them; integrate the messages in their texts or separate it into the private app. From day one I was up and running with my phone. I use it every day.

**What do you dislike about Spruce Health?**

I wish that I had different sections for the messages and the faxes. I dont want them to be mixed together. I hope to get to learn more about using the tags to more efficiently send information to my groupings of clientele.

**What problems is Spruce Health solving and how is that benefiting you?**

Not totally sure how to answer this. I know they are working with neurodivergent people and putting on meetings or trainings where the presenters are offering us information on running a business that has good boundaries, expectations, and using behaviors like pacing, chunking and prioritizing to be as successful as possible within our own abilities. I feel very empowered.

Spruce is very convenient and very easy to use. It provides what I need to communicate with clients and it is HIPPA compliant. It is easy to use, I can store my clients contacts with pertinent information, and it allows for sending of documents. This is a great service for a very reasonable price.

It took a long time to get permission to send text messages even after paying for that add on service.

This platform has so many great features at the lowest cost tier, which has been helpful as an individual practice owner. I can track notes, use templates, and tag conversations. I’m likely not using more than half of the features found on Spruce and it is significantly better than the last 2 services I've used for phone services in the last year and a half!!

**What do you dislike about Spruce Health?**

My biggest dislike would be that some of the features require a higher tiered subscription, like auto replies to missed phone calls, for instance.

I really needed a place that felt secure for my clients to communicate with me. It keeps contacts separate from my phone contacts. And it allows me to keep notes of conversations or contact with clients in the conversation, which means I can track that immediately and transfer to my EHR when I’m able to do it all at once.

My EHR already has a secure portal, but it is a great feature that I would likely also be using more of in Spruce if mine did not.

**What do you like best about Spruce Health?**

I like that Spruce Health offers fax, messaging, client secure messaging, video and phone options. It's fairly easy to use. Most customer support is done via the app or in the website portal. It was easy to set up although I am in private practice by myself. While it's not a "one stop shop," it does the job and it's affordable. To my knowledge it does not have third party API integration with EHR's like Simple Practice. I use it daily. I know you can pay people to set up the phone trees and other items for large organizaitons.

**What do you dislike about Spruce Health?**

The biggest downside here is that clients MUST make an account for messages to be HIPAA compliant. I get clients who text the number but won't make an account. I then can't respond back because it's not HIPAA compliant. Also, when they do make an account, any messages they sent prior do not show up for easy archiving. I can send an invite but many do not make an account limiting it's functionality. Paubox has introduced a feature where they can text the number w/o making an account. Paubox does automatic end to end encryption in their app. I have not used it frequently as I use chart chat if they won't make an account. 

My second wish is that they added e-mail for a "one stop shop." Integrating with an EHR would be great but I know third party API has it's own challenges.

It offers phone calls, video, messaging and fax via app or the website. It's compatible with my iPhone, Android Tablet and Windows based computer.

The only thing I don’t like is that Psychology Today notifies me in Spruce every time I get a call which is redundant and adds extra logs to the thread and makes it confusing for me.

Spruce was a decent unified platform—text, fax, voicemail—all under one roof. That’s where the compliments end.

**What do you dislike about Spruce Health?**

Their support is non-existent, and their “chat” is a useless AI wall. I tried to send a private support message voicing real frustration—like a normal clinician running a busy practice would—and their platform blasted it to my entire clinical team. My words, intended for Spruce Support, were sent to everyone. It was humiliating, morale-crushing, and completely avoidable. No warning. No safeguard. No sense of what “professional discretion” even means. No one at Spruce has responded in any meaningful way. It feels like a tech company that’s scaled for money, not care.

Spruce Health feels like a cautionary tale of what happens when you mix bad UX design, poor customer support, and zero accountability into something meant to support mental health professionals.

I’m a licensed therapist running a full team. When I reached out for support in frustration—calling out the lack of human contact—I never imagined their system would distribute my private comments to my entire staff. The emotional damage and embarrassment were real.

Spruce Support? There is none. Just a chatbot and silence. It’s 2025—if you’re going to handle HIPAA-sensitive communications, you’d better understand professional boundaries and basic logic. This isn’t just bad software—it’s organizational negligence disguised as a tech solution.

**What do you like best about Spruce Health?**

Nice intuitive flow, great customer service, and EMR integration. I currently have the basic, and wish they had a “try it before you buy it” option to see what the higher tier provides. But otherwise, the features of even a basic account are plenty to efficiently and effectively run my medical practice. I’ve had a few telemedicine calls and those went well, good quality video and audio. I just started getting into the saved messages feature, but like it so far.

As above, I do wish they gave basic tier clients a trial of the “Communicator” level to see if the upgrade features are worth doubling the price.

It’s a small area for improvement in an otherwise outstanding platform, but if Spruce added this functionality, it would take the experience to an entirely new level.

Spruce Health has been a tremendous solution for us, both in our medical practice and in our medspa. It has solved the challenge of keeping patient and client communication organized, secure, and efficient. Before Spruce, we were juggling multiple platforms and constantly worried about missed messages or delays. Now everything—calls, texts, telehealth, and team messaging—flows seamlessly in one place.

The integration with Elation has been especially valuable on the clinical side, keeping documentation and communication in sync and reducing the risk of errors. The ease of use of Spruce, not only for myself and other providers but especially for even my most technically challenged patients is what I love most about Spruce. having all my communications in one place keeps me organized.
I love the tagging feature to assign to other providers.

Some small things such as  being able to have the business logo on fax cover sheets and being able to customize the face sheet with page count would be great. 
A subnote under patient name that's visible with all interactions (to identify if it's the parent or guardian of a patient, or nickname).

Lastly, it would be nice if there were a way to have a sub account type for a lesser amount of money. Id like to communicate with my billing and accounting staff in Spruce to allow for use of patient names but there's no need for them to have full use of the Spruce services.

I love that Spruce is customizable to my practices needs.  I credit spruce with reducing our admin time and allowing us to manage our practice with less admin staff. I find their support to be great. Latoya Irving who has been helping us make some updates recently has been extremely helpful for our practice and has been very responsive.  Our set up on Spruce Health allows patients to make and cancel their own appts through the app by directing them to our hippa compliant forms and scheduling website. It also allows them to request refills without needing our staff to go through hundreds of phone calls for refill requests.  Patients can also update their demographics there.  When patients have appointments we have it assign it to their provider when they click their providers name, which alerts their provider that the patient is there and from there they can fill out screening tools. Spruce Health has taken a large administrative burden off of our practice. Spruce can be as simple or as specific as you'd like to make it.  Highly recommend.

I would love if spruce had integrated HIPPA compliant forms of their own. I'd love if we could have more than one person on the same video call which I know they are working towards! Also would love if we could use a practice backdrop behind us during the call. These are all small things. Spruce is a great match for my team of therapists and our virtual assistant. I love that we can 'tag' our VA privately in messages when we are ready to hand off a client to her scheduling or billing abilities. And being able to assign message threads to a specific clinician or caseload keeps the feed from clogging up and distracting everyone unnecessarily. It's also been easy to add or delete users as my team changes and grows.

In the beginning, when my team and I were sending 'internal' messages (not meant for client's eyes) we make lots of accidents and accidentally send them to the client instead! But we quickly learned that we could set our default messaging preference to 'internal' - now, no clients ever accidentally see internal messages because we have to intentionally choose to communicate in the client field every time.

The most helpful thing about Spruce is the support. Always accessible and currently not AI directed which is very appreciated. Always quick to answer with understanding and kindness and open to helping not only in various platform situations, questions, but also in payment and financial situations. I believe that for me as a sole provider organization became a problem as someone who began with very few patients 4 years ago. Forward to now, the long list of wonderful patients and professional contacts along with new patient calls adding to multiple calls of other kind became overwhelming. I do not have an extra person to help me organize them. I began on the platform when it operated in a different way and my organization was haphazard. As Spruce grew and evolved at a level I did not really predict, more organizational methods were developed. Unfortunately, with limited time, I left my account usually as it was and organization became more difficult. I think the organization is or can be easily done with tags and making lists, etc.,  I think I am beyond help! Or simply need help of another person. I did ask about this concern and the support staff was great. I did get directions and explanations of uses of the different methods to organize and am aware where they are and that thry are accessible. Now partly using some methods but without a good flow, I still over-tag and am a mess. I am an over-organizer - meaning that in the end am not organized at all. I believe I would love it if there were workflows to organize easily focused  around established practices and long lists. Also - signing in and not seeing the inbox of everything at once at first glance and be barraged by messages and phone calls immediately would help for those of us that are more sensitive and overwhelmed easily.

The user experience for the practitioner and the client is lacking. If you are a small business that is trying to learn so much including how to run taxes, an office, staff, etc, then Spruce is a headache. Unless you plan on using the most basic features and not learn much else, don't count on support to be excited to help you explore the rest of the capabilites that are available on the lower plan. 

Instead, I was shuffled to about 10 different videos and articles,and told to attend their "Onboarding Session," which I was beyond. I was told I could spend time asking questions, which I did, but it wasn't enough to work out the kinks. They said I could attend another onboarding session to ask more questions after they lectured about the basics.  Asked numerous times for additional one-on-one help, but was denied.

In a nutshell, there isn't a choice for practices who are not big but not small in their customer service world.

HIPAA compliant messaging. Ease, you can hop on your phone and it is done. Whereas going to a portal and logging in is not as user friendly. I can share documents easy. However, setting it up to use for my practice is messy, hard to understand.

Program needs to add group capabilities.
Update 5-14-24:  I've stopped using the video conferencing platform because it is such a bandwidth hog that my clients are unable to obtain enough bandwidth for it to function.  And there is still no group capability, which I NEED.  I've switched to ZOOM for video conferencing to get this feature, and for more stable connections.

Good product for a good price.  If you need HIPPA compliant texting, this meets the need and is relatively easy to use.  The refresh rate on some screens on my phone is a little slow, but the web version is generally timely.

Spruce solves the problem of HIPPA compliant texting for clients who prefer this method of communication.  I particularly like having records of my calls/video chats in the same stream as the text, so they appear in context.

**What do you dislike about Spruce Health?**

I don't like that calls have to come through my phone number. I know people can't see my personal number but it makes it a little more complicated. It just shows my spruce number as an incoming call so I don't actually know who is calling me until I answer.
