I really appreciate the 'easy to use' aspect of InterSystems IRIS for Health. I like the ability to process and route HL7 messages efficiently. Although it's initially a bit tricky to find everything, over time all the functions and features become easier to use.

Even though it's tough and robust, I think the UI could be a little more simple. Maybe to be more 'Modern' designed, and better readability. At first it's a bit difficult to understand where everything is located, but with the right time, all the functions and features it gets easier.

What I like best about InterSystems IRIS for Health is how it brings data integration, interoperability, and analytics together in one platform. It makes working with healthcare standards like FHIR and HL7 much easier while still being fast and scalable. I also appreciate the built-in tools for real-time data processing, which reduce complexity and speed up development.

**What do you dislike about InterSystems IRIS for Health?**

One thing I dislike about IRIS for Health is the learning curve, especially for developers who are new to the InterSystems ecosystem or to ObjectScript. Some concepts and tooling feel quite different from more mainstream stacks, which can slow onboarding. I also find that documentation can be fragmented, making it harder to quickly find practical, end-to-end examples.

InterSystems IRIS for Health is solving the problem of healthcare data silos and complex interoperability. Healthcare data comes from many systems, formats, and standards, and IRIS for Health provides a single platform to ingest, normalize, route, and store that data using standards like HL7 and FHIR.

Very easy to use, very intuitive, and has great support from a developer community that makes projects much easier.

Although many automated processes do not require extensive knowledge, the security configuration is quite complicated.

**What problems is InterSystems IRIS for Health solving and how is that benefiting you?**

Integration with third parties is very simple, and large operations can be carried out with minimal development time. We have integrated 20 laboratories, securely storing analytical results and ensuring their confidentiality.

**What do you like best about InterSystems IRIS for Health?**

I like the ability to quickly manage data formats like HL7 or FHIR in the healthcare world and also to support other more common types of integration like SOAP or REST. It is particularly simple to manage and quickly create REST through the platform and its management portal. I also greatly appreciate the database capabilities, in fact, it is not just an enterprise service bus but also a database management system. The InterSystems IRIS for Health database is very efficient thanks to the use of globals, the data format in which everything is saved in the IRIS DB.

**What do you dislike about InterSystems IRIS for Health?**

I believe that in general the documentation is somewhat lacking regarding the InterSystems world, although they have been improving a lot on this point lately. What can certainly be improved is the user interface, which seems a bit outdated. Moreover, there are many functions available today but not much utilized by users, such as the entire business intelligence sector or that of machine learning and embedding vectors.

**What problems is InterSystems IRIS for Health solving and how is that benefiting you?**

With InterSystems IRIS for Health, it is very simple to create highly performant and fast-to-develop HL7 or FHIR integrations, thanks to the use of native classes developed specifically to handle data types in HL7 or FHIR formats. Additionally, it is very simple to create REST integrations. I also greatly appreciate the efficient database in data management, which recently also supports embedding vectors.

**What do you like best about InterSystems IRIS for Health?**

I'm surprised to say, the Ensemble (now Integration) Productions. When I first started learning about them, I thought they would either be 1) hard to use or 2) very slow performing due to the fact they use a browser-based GUI setup system instead of code. Now that I've used them quite a bit, I find them an indispensable tool for my job, and they've solved several long-lived issues.

**What do you dislike about InterSystems IRIS for Health?**

Licensing and cost-of-entry. InterSystems has (if you can find it) an installer for Raspberry Pi hardware which actually performs quite well on these tiny little computers. I would love to be able to purchase a personal license for the technology as I have several projects around the house that would be much easier to implement using this technology instead of having to "roll my own." I've asked many times how to purchase a valid single-user license, and I've never received a response. There's millions of Raspberry Pis across the globe, it would be phenomenal (IMHO) to have a tool like this available on a small but very capable computer.

**What problems is InterSystems IRIS for Health solving and how is that benefiting you?**

It has solved several data collection and data gateway problems that previously used external (and ancient) tools to accomplish. It was nice to be able to bring that functionality back under the InterSystems technology to make improvements and maintenance of our systems easier.

I think that there are many advantages
1. the support is extraordinary, whether is a bug in the product or misconfiguration or even bugs in the infrastructure (kubernetes) the staff is always eager to help especially Tani and Ari 
2. There is an option to create custom operation where you can enhance the FHIR capabilities, which was really helpful, since we needed it to support resource relationship and search options that are not FHIR native
3. the product has a built in ETL tool where you can build an entire ETL / ESB that allows advanced logic and transformation
4. our license includes the Fhir SQL builder, which enable you to configure sql views on top of the data without duplicating it, this allows you to query the data in an SQL language.

**What do you dislike about InterSystems IRIS for Health?**

1. backup process is a bit clumsy, you need to freeze and thaw the db for the backup
2. mirroring needs more adjustments like to mirror the security as well
3. there are some hidden features like enabling logs that needs to be broadcast

We were require to implement FHIR as part of a regulation law, and InterSystems IRIS for Health was the best fit for us, since it allows customization of the build in operations and it has built  in features that allow us do transform the data on route

**What do you like best about InterSystems IRIS for Health?**

I use IRIS for Health almost every day, and what I like most is how much it simplifies working with healthcare data. Things like HL7 and FHIR, which are usually a headache to deal with, are much easier here because the platform handles a lot of the heavy lifting.

It’s also great that everything you need is in one place — database, integration engine, message routing, and even analytics. Once you're familiar with the environment, it’s pretty straightforward to implement solutions. This really speeds up development and reduces back-and-forth between systems.

The customer support from InterSystems is generally responsive and helpful, especially when it comes to urgent production issues. They do take healthcare seriously, so you usually get solid answers.

For more technical or niche problems, the turnaround time can vary, and sometimes it feels like you’re waiting on engineering or digging through documentation yourself. There’s not as much community content or Stack Overflow presence compared to other platforms, so you're mostly relying on official channels. It would be great to see more real-world examples and quicker resolution on complex tickets.

The main downside is the learning curve. If you’re new to IRIS or ObjectScript, it can feel unfamiliar and a bit old-school compared to more widely-used languages like Python or JavaScript. It takes time to get comfortable.

Setting up your first few productions or integrations can feel a bit overwhelming without the right guidance. Also, since it’s a licensed product, cost and access can be a concern — especially for smaller teams or if you're trying to build a quick prototype. More beginner-friendly examples and community support would really help ease adoption.

The built-in tools for message routing, transformations, and monitoring are very handy. You don’t need to use separate software for each function — it’s all there in one place. That said, the initial learning curve can slow you down a bit if you're new to the InterSystems way of doing things, but once you’re used to it, integration work becomes pretty smooth and repeatable.

InterSystems IRIS for Health is solving one of the biggest challenges in healthcare IT — integrating multiple systems that all speak different data formats and protocols like HL7, FHIR, JSON, and XML. In a typical hospital environment, there are many moving parts: lab systems, electronic medical records (EMRs), pharmacy systems, radiology modules, and more. Each of these systems may use different standards or interfaces, making communication between them complex and time-consuming to manage.

IRIS for Health simplifies this by providing a unified platform with built-in capabilities for parsing, transforming, and routing healthcare messages. Instead of spending hours writing custom code or managing multiple third-party tools, we can quickly set up and maintain interfaces directly within IRIS. This reduces development overhead and increases consistency across systems.

Another major benefit is how IRIS centralizes our workflows. If you’ve ever worked in healthcare IT, you know how messy and fragmented data can be. Getting different systems to talk to each other  lab systems, EHRs, imaging platforms can feel like trying to get a group of people who speak different languages to sing in harmony. That’s where InterSystems IRIS for Health steps in.  This platform was built with healthcare in mind. It’s not a general-purpose tool trying to wedge its way into the industry. IRIS for Health is specifically designed to handle the complex world of clinical data, and it does it well assuming you’re ready for a bit of a learning curve. It is Fast and Scalable. FHIR Made (Relatively) Easy. Data analytics is also very easy here as the data is stored in a very structured manner.

Iris for health is a strong has a really really strong DB engine with a powerful and fast database. It is really robust, it always starts or at least gives you the right hint on what isn't working. It's versatile thanks to its multiaccess model and it is really really fast.
Moreover the "for Health" edition gives you a lot of classes to manage the usual messages and helps you to manage a wide range of standard protocols in healthcare like fihr, hl7 and so on.
The customer support is really fast, competent and helpful.

what is missing is a good replication engine on mirror pair. The data are replicated, but the settings are saved on the local instance and are not replicated

As a developer and system administrator in the healthcare sector, I've been using the product for many years and I think the strengths of InterSystems IRIS for Health are it's robustness, performance and an easy to use,rich healthcare standards implementation (e.g. HL7 v2.x, DICOM).

**What do you dislike about InterSystems IRIS for Health?**

It requires a significant amount of effort to achieve a comfortable level of proficiency with the product. For beginners, the MUMPS legacy, together with object oriented concepts in ObjectScript (such as multiple inheritance), may be daunting. Though embedded Python, a recent addition, may alleviate this, the product roots are still very present.

**What problems is InterSystems IRIS for Health solving and how is that benefiting you?**

I've been successfully implementing the following in two medium sized (approx. 1000 beds)  hospitals  in Belgium using InterSystems IRIS for Health :

- patient demographic and admission/discharge/transfer data, transforming and filtering
- appointment data transforming and filtering, following patient consultation pathway
- document processing : route document (reports, results, ...) from various sources to document archive, and reference them in health records
- master file management and distribution (practitioners, procedures, locations, etc.)
- patient consultation registration workflow, mixing machine (kiosk) and human (desk) interactions
- interfacing with third party APIs (SOAP, REST)
- interfacing with medical devices (monitoring systems, DICOM modalities, ...)
- interfacing with legacy systems such as IBM DB2 database

I appreciate how the UI is very intuitive, meaning it's easy to navigate and visually pleasing. The new UI is great because I am able to see the data transformations and rules now while viewing the production configuration page. I think this aids in interface overview. I find it easy to build interfaces on here and manage them. Also, anytime I have issues, their tech services team is really helpful.

**What do you dislike about InterSystems IRIS for Health?**

While the new UX is really helpful for viewing big picture stuff in the production configuration page, I generally find it more difficult to navigate.

**What problems is InterSystems IRIS for Health solving and how is that benefiting you?**

The greatest benefit of IRIS for Health is being able to do so much from the System Management Portal. other applications don't give users this kind of power when building and managing interfaces.

As a naive user, I struggled to learn the platform quickly

**What problems is InterSystems IRIS for Health solving and how is that benefiting you?**

Modern Trials needs to search for patients who meet specific clinical trial eligibility criteria (sometimes 50 or more) from EHR data. This data comes from many different healthcare organizations which means it comes in different formats and structures, and uses different standards, units, conventions and terms. However, for that data to be useful, it must be all made completely uniform. This monumental task is a major challenge when trying to utilize health records from different sources, particularly at scale with millions of records.

Everyone we work with at InterSystems is both knowledgeable and helpful. Even if they don't know the answer to a question, they are more than happy to pull in the right resources to help answer your question. This has made our initial setup of IRIS and our cutover with currently integrated customers a breeze.

**What do you dislike about InterSystems IRIS for Health?**

Every so often, we are surprised with some of the issues we run into. As an example, we've had quite a few issues setting up Source Control. Throughout that time, InterSystems has been more than happy to meet with us and work through the issues, but it was a little surprising that we were the first group to encounter some of the issues.

**What problems is InterSystems IRIS for Health solving and how is that benefiting you?**

IRIS is helping us scale. We are switching from Mirth to IRIS to provide a better platform for our team to work from and make it simpler to integrate with other organizations.

**What do you like best about InterSystems IRIS for Health?**

By implementing InterSystems IRIS for Health, YCare achieved rapid and efficient database management alongside FHIR-based healthcare data exchange, all within a single package. This significantly enhanced our efforts toward achieving interoperability and integration with multiple EHR systems. Additionally, the platform is optimized for AI applications, including advanced RUG and vector search functionalities.

**What do you dislike about InterSystems IRIS for Health?**

InterSystems IRIS for Health does not fully support the convergence of all modern data frameworks. As a result, we had to re-engineer certain subsystems to ensure full compatibility with the platform.

**What problems is InterSystems IRIS for Health solving and how is that benefiting you?**

We are partnering with the University of Florida Healthcare's Cardiology Department on a medical trial to monitor patients with LVAD devices post-surgery. To conduct the trial over a six-month period, we require seamless data exchange integration with UF Health's EPIC EHR system. We are collaborating with InterSystems engineers to establish bi-directional integration between EPIC and the YCare Digital Health Platform, ensuring smooth and efficient data flow.

Intersystems provides a robust solution for the industry. There's lots of information, documentation, videos, and online learning to discover everything there is about each of their products.

All of their products can be set up to work right out of the box, and customization is available to meet anyone's business needs. 

Their WRC and technical support are thoroughly knowledgeable and quick to respond.

Some of the naming conventions cam be confusing, but do not let that stop you.

**What problems is InterSystems IRIS for Health solving and how is that benefiting you?**

Integrating disparate systems into one cohesive supportable platform. As a long time consultant to the Veterans Adminsitration, IRIS for Health has addressed some of the long standing challenges of working across the largest health care enterprise in the country.

Using IRIS for Health allows for easy integration with most laboratories and medical institutions, facilitating secure data transfer. When our partners and potential clients learn that we use IRIS for Health, it significantly boosts their trust in us and greatly increases the likelihood of starting a partnership. Additionally, the IRIS for Health support team provides excellent consultation quality. Currently, we have been using IRIS for Health for a year and have no complaints.

Difficulties in initial circuit design require the hiring of additional staff, as our developers are unable to cope on their own.

We use Healthshare Health Connect and FHIR DB as building blocks for our solution for cross-organizational booking platform for clinics in Israel. What our CTO really liked is the out-of-the-box solution for most of our architectural challenges:
1. Integration suite with EMR
2. Visual integration builder with a mix of format and dynamic fields mapping: HL7v3, C/CDA, FHIR, free text
3. Ready-to-go FHIR DB

As complex product it definitely has its learning curve, but I believe its unevitable

We had to integrate with various proprietary EMRs to create "medical booking showcase".

IRIS for Health allow us to be interoperable with any Hospital Information System

great integration engine for intergrating healthcare, lots of good features to help map messages and develop interfaces as well as maintain the system, great security and frequent updates to make working on it easier.
Interfaces are easy to implement with nice front end to work with.
great suppoer for dev team and intersystems website as well as the community.

**What do you dislike about InterSystems IRIS for Health?**

the IDE is out dated, sometiems its hard to cade compared to Visual studio IDE whenre it propmpts you code, this would be a great new feature for cache.

**What problems is InterSystems IRIS for Health solving and how is that benefiting you?**

helps to integrate different hospitals and theirsstems together allowing them to communicate?

We liked the scalability and versatality that is provided by platform.

**What do you dislike about InterSystems IRIS for Health?**

Being startup and with very limited bandwidth that we've, we found this solution to be little more resources intensive. But again we're startup, for most of organization of mid-large size this will never be an issue.

**What problems is InterSystems IRIS for Health solving and how is that benefiting you?**

Its' helping in solving informatin exchange which is biggest hurdle for us.
