Billibles are highly unpredictable even when you use the calculator. Usage is harder to predict and perhaps its a matter of experience. However we've found with enviroments utilized by 100 users on a cloud server its night on impossible to 100% tell a client what their bill may be even using historical predictions. The only downside I'd say there was for me when using Application Gateway, is you need to 100% have a domain name setup in advance and ready to use. Most features don't play well with just an IP and you can't use the azurewebsites domain either which makes sense.
