Survicate has been widely used in my organization. It easily integrates with slack and also has a clean UI for making surveys

Survicate is a pretty straightforward and easy to use survey tool. This allows product, marketing and CS teams within our company to set up and run their own surveys with little to no assistance from the market research team. It doesn't allow very complex routing or intricate quotas, but for simple to moderate complexity surveys it's great! Also, we can easily integrate it with internal systems, so we can extract and report on NPS number in a single platform. Also, their customer support is very quick at responding and helpful.
