This form for feedback. You need to realize that filling a form takes time. I was willing to fill it but once I saw "required" large input boxes, my motivation died. If you want feedback, make it more focused, ask me specific questions and ask me to rate it. Do not put the work on the customer to figure out how and what they should give youo feedbakc for.
