# What is the easiest way to access inputting diagnoses?

It depends from the type of way you are accessing (Chart Review, Encounter, Office Visit, etc.) You can view the diagnosis from the Plan Activity Tab and click on Visit Diagnosis. If you are not in an encounter you can go into Chart Review and go to that encounter date and see what diagnoses were entered for the visited.

if sending to another physician via a referral, there is a field called dx/px; where you can either free text the diagnosis / procedure, or use one of the ICD codes to determine the diagnoses (ICD-10 imported list).  

(this is based on a referral and not via the physician entering the information).    I am not familiar with the clinical side, as I&#39;m certified cadence, prelude, referrals, welcome.  Thank you
